The faucet coupler assembly was worn out and would always let go when I turned the faucet on.

Multi-tip screw driver, vice grips
Repair & Advice
You'll need to take off the side panels of the washer to feed the hoses through. After that, for easier access to where hoses attach, tip the washer onto its back and remove the floor panel.
